The term ‘transition’ for children and young people with Special Educational Needs and Disabilities (SEND) refers to the planned and supported movement from childhood into adulthood, encompassing changes in education, health, social care, and life circumstances. It is simply the process of moving between different stages of education or care services.
 
Livewell and Plymouth City Council have produced this poster with links to some resources to help you and your family members with transition to adulthood.
